[18F]-FTC-146

[18F]-FTC-146 is a highly selective radiolabeled ligand for the sigma‑1 receptor (S1R), labeled with the positron-emitting radionuclide fluorine‑18. It is used as a diagnostic radiopharmaceutical for positron emission tomography (PET) imaging, enabling visualization and quantification of S1R distribution in vivo. The compound demonstrates over 1000-fold selectivity for S1R over sigma‑2 receptors and vesicular acetylcholine transporters, making it an excellent tool for noninvasive imaging of S1R in various tissues. Preclinical and first-in-human studies have shown that [18F]-FTC‑146 is safe, well-tolerated, and has favorable pharmacokinetics. Its primary clinical applications under investigation include neuroinflammatory diseases, chronic pain conditions such as complex regional pain syndrome (CRPS), sciatica, neuralgia, radiculopathy, spinal cord diseases, nociceptive pain syndromes, spinal claudication, fragile X syndrome research models, and tumor imaging[1][2][3][4][5][9][10]. The agent is not intended to induce physiological or pharmacological effects but serves as a molecular imaging probe.
